For new applicants in the Adaptive Riding, Barn Pals, and Veterans Horsemanship Programs. The Medical Release Form must be completed by the applicant’s healthcare provider.

Annual Forms

All participants must submit an Annual Participant Update Form each year. Participants with Down Syndrome must submit the Annual AAI Medical Release Form each year.

Other Applications

• Group Visit Participant Application (For special education classes, groups for individuals with disabilities, veteran groups, etc.)

• One-Time Ride (For individuals diagnosed with a terminal illness)

• Scholarship Form (Thanks to our generous donors, Ride for Joy is able to provide a limited number of scholarships to those facing financial hardship.)
